Please find an application form, personal health history survey, and a liability release/consent for treatment, found on this web site under ApplicationForms. All forms must be completely filled out, signed and returned to SFL before your participation in any event can be confirmed.
For your own health and safety, Ski for Light requires all participants,
sighted or not, to
complete the attached personal health history form. This confidential
record requires your
candid, accurate and legible completion. Every item in every section
must be completed. Mark
N/A if any section is not applicable to you.
Anyone in average physical condition can expect to complete these events.
If we have any
questions about your ability to participate we will call and discuss
the matter with you. In
questionable cases the decision to participate will be that of the
participant and his or her
physician. Participants must take responsibility for control of any
health problems. For example,
those with insulin dependent diabetes must monitor their blood glucose
levels daily. We really
need you to take a high level of personal responsibility for your health
and well being at this
event not only for your own safety but for that of others. Your enjoyment
will also be enhanced
by any measures you take to improve your physical fitness and stamina
before the event.
Although Guide Dogs are welcome on the trips, there are restrictions
that may eliminate your
participation in the Sunday activities. On Sundays (after checkout
time in the AM) you will be
responsible for attending to your dog, as there are no local kennels.
Please consider this when
deciding to bring your dog or not.
Transportation will be available by car-pooling. You will be contacted
about a week prior to
departure to make travel arrangements. Guides are encouraged to let
us know if they are willing
to drive and how many passengers they will be able to take. All passengers
are expected to
"chip in' for gasoline.
Please note that if you are coming to an event from out of town, DO
NOT make your travel plans (air flights, train, bus, etc.) prior to 7:00
PM on Sunday. It is very difficult to get back to
transportation centers prior to this time without disrupting many other
participants.
